What to Expect During Your O-Shot Procedure: A Step-by-Step Guide

Anticipating any medical procedure can create anxiety, particularly one involving such an intimate area of your body. Understanding exactly what happens during your O-Shot treatment—from the moment you arrive until you leave—can transform that anxiety into confident anticipation. Let me walk you through every step of the process as it unfolds at Biltmore Restorative Medicine, so you know precisely what to expect.

Before Your Procedure: The Essential Consultation

Your O-Shot journey begins well before the actual procedure day with a comprehensive consultation. This isn’t a brief, transactional meeting—it’s an in-depth conversation where we explore your concerns, health history, goals, and expectations.

During this consultation, we discuss your specific symptoms in detail. How long have you experienced these concerns? How do they affect your daily life and relationships? What treatments have you tried previously? Understanding your complete story helps me determine whether the O-Shot is right for you and what realistic outcomes you can expect.

We review your medical history thoroughly, including current medications, past surgeries, hormonal status, and any conditions that might affect your candidacy or response to treatment. This is also when we discuss your goals—what would meaningful improvement look like for you? What aspects of sexual wellness matter most?

I explain the procedure in detail, including how it works, what sensations you might experience, potential side effects, expected timeline for results, and any restrictions afterward. This is your opportunity to ask every question on your mind. No concern is too small or too personal.

At Biltmore Restorative Medicine, we approach sexual wellness comprehensively. During your consultation, we may also discuss hormone levels, nutritional status, stress management, and other factors that influence sexual function. This whole-person perspective helps us create a treatment plan that addresses your needs completely, not just in isolation.

Preparing for Your Procedure Day

Once we’ve determined the O-Shot is right for you and scheduled your procedure, minimal preparation is needed. We typically recommend:

Hydration: Drink plenty of water in the days before your appointment. Good hydration improves blood draw success and PRP quality.

Avoid Blood Thinners: If possible and with your physician’s approval, avoid aspirin, ibuprofen, and other blood-thinning medications for several days before treatment to minimize bruising. Never stop prescribed medications without consulting your doctor.

Schedule Wisely: Plan your appointment when you won’t need to engage in sexual activity for 3-4 days afterward. Avoid scheduling immediately before important events or occasions.

Eat Normally: Have a normal meal before your appointment. There’s no need to fast, and having food in your system helps prevent lightheadedness during the blood draw.

Arrive Relaxed: Give yourself plenty of time to arrive without rushing. Bring your questions list if you have any remaining concerns.

Step 1: Arrival and Initial Preparation (10 minutes)

When you arrive at our office, you’re welcomed into a comfortable, private environment. We understand this procedure involves vulnerability, and we’ve created a space where you can feel completely at ease.

After checking in, you’re taken to a private treatment room. You’ll be asked to change into a comfortable gown, similar to what you’d wear for a gynecological exam. We provide privacy for changing and ensure you’re comfortably positioned before beginning any steps of the procedure.

This is also when we review your consent form one final time, answer any last-minute questions, and ensure you feel completely comfortable proceeding. Your comfort and confidence are paramount—if you have any hesitation, we address it before moving forward.

Step 2: Application of Numbing Cream (5 minutes)

Comfort is a top priority during the O-Shot procedure. To ensure you experience minimal discomfort during the injections, we begin by applying a pharmaceutical-grade topical numbing cream to the treatment area.

This cream contains local anesthetics that penetrate the tissue and block pain signals. We apply it generously to the clitoral area and the areas around the vaginal opening where injections will occur. The application itself is completely painless—it simply feels like cream being applied.

Once the numbing cream is in place, we cover it with a protective dressing to enhance penetration and effectiveness. This is when your wait begins—the numbing cream needs 20-30 minutes to take full effect.

Step 3: The Waiting Period and Blood Draw (20-30 minutes)

While the numbing cream works its magic, we move to the next essential step: drawing your blood. Using a small needle similar to routine blood tests, we draw approximately 10-20 milliliters of blood from your arm—about 2-4 teaspoons.

This blood draw feels just like any standard blood test you’ve had. You’ll feel a small pinch as the needle enters, then perhaps slight pressure as blood fills the collection tube. The entire draw takes less than a minute.

Some patients experience mild lightheadedness during blood draws. This is why we have you seated comfortably and encourage you to eat before your appointment. If you have a history of feeling faint during blood draws, let us know so we can take extra precautions.

During this waiting period, you can relax in our comfortable treatment room. Some patients bring books or use their phones, while others simply rest. We’re nearby if you have questions or need anything.

Step 4: PRP Processing (10-15 minutes)

While you continue relaxing with the numbing cream in place, your blood undergoes processing to create Platelet-Rich Plasma. This happens behind the scenes, but understanding the process helps you appreciate the quality care you’re receiving.

Your blood is placed in specialized tubes containing anticoagulants to prevent clotting. These tubes are then loaded into our centrifuge—a sophisticated device that spins the blood at precisely calibrated speeds.

The centrifugation process separates your blood into layers based on density. Red blood cells settle at the bottom, platelet-poor plasma rises to the top, and in between is the golden layer we’re after—the platelet-rich plasma containing concentrated growth factors.

At Biltmore Restorative Medicine, we use state-of-the-art centrifuge systems with carefully controlled speeds and timing to achieve optimal platelet concentrations—typically 4 to 8 times the normal concentration found in whole blood. This quality and concentration directly impacts your results, which is why we invest in the best equipment and maintain rigorous protocols.

The technician carefully extracts the PRP layer, ensuring we capture maximum platelet concentration while minimizing red blood cell contamination. The result is a golden-colored substance packed with healing potential.

Step 5: Removing the Numbing Cream and Final Preparation (5 minutes)

After the 20-30 minute numbing period, we gently remove the numbing cream and clean the area. By this point, the treated area should feel significantly numb—many women report feeling minimal to no sensation in the numbed areas.

We assess your comfort level by gently touching the area. If you feel more than light pressure, we can apply additional numbing cream and wait longer. We never proceed until you’re adequately numb and comfortable.

Your PRP is now ready, drawn into syringes with very fine needles specifically designed for delicate tissue. Everything is prepared for the actual injection phase.

Step 6: The O-Shot Injections (5-10 minutes)

This is the moment you’ve been anticipating, and I want to assure you—with proper numbing, this step is far more comfortable than most women expect.

We inject the PRP into specific, strategic locations:

Clitoral Injection: A small amount of PRP is injected around the clitoris. The numbing cream has made this area quite insensitive, so most women feel only slight pressure, not pain. This injection enhances blood flow and sensitivity to the clitoral tissue.

Anterior Vaginal Wall Injection: This is where we inject PRP into the anterior (front) vaginal wall, including the area commonly called the G-spot. Using a speculum for visualization and precise needle placement, we deliver PRP to this highly innervated area. Again, with adequate numbing, you’ll feel pressure and perhaps a sensation of fullness, but typically not sharp pain.

Urethral Area Injection (if applicable): For women concerned with urinary incontinence, we may also inject PRP near the urethra to strengthen supporting tissues. This additional injection follows the same comfort principles—well-numbed and carefully placed.

The entire injection process takes only 5-10 minutes. Throughout this time, I communicate with you about what I’m doing and check in about your comfort level. If you experience significant discomfort at any point, we can pause and address it.

Most patients are surprised by how comfortable the procedure is. The anticipation is often more challenging than the actual experience. With proper numbing and experienced technique, discomfort is minimal.

Step 7: Immediate Post-Procedure Care (5-10 minutes)

Once the injections are complete, we give you time to rest for a few minutes. Some women feel slightly emotional—this is completely normal. You’ve just taken a significant step toward reclaiming your sexual wellness, and emotions can surface.

You may notice some immediate sensations in the treated area—slight fullness, mild tingling, or enhanced sensitivity. These are normal responses to the injection and the PRP beginning its work.

We’ll check for any immediate concerns, ensure you’re feeling well, and help you get dressed when you’re ready. There’s no rush—you can take whatever time you need to feel composed and comfortable.

Before you leave, we review post-procedure instructions thoroughly:

  • Avoid sexual activity, including orgasm, for 3-4 days
  • You may experience minor swelling, bruising, or tenderness
  • You can use ice packs if desired for comfort
  • Resume normal activities, including work and non-vigorous exercise
  • Contact us immediately if you experience signs of infection or concerning symptoms

We schedule your follow-up appointment, typically 4-6 weeks after treatment, to assess your progress and response.

The First 24 Hours After Your O-Shot

Understanding what to expect in the hours and days following your procedure helps you feel confident and prepared.

Immediate Hours: As the numbing wears off over the first few hours, you may notice mild tenderness or sensitivity in the injection sites. This is normal and typically quite manageable. Over-the-counter pain relievers can help if needed, though most women find them unnecessary.

You might notice slight swelling or a feeling of fullness in the treated areas. This is actually a positive sign—it indicates the PRP is in place and your body is beginning its healing response.

First Night: Sleep position doesn’t require modification, though some women find sleeping on their back more comfortable. The tenderness, if present, is usually minor and doesn’t interfere with sleep.

Next Day: Most women return to all normal activities the next day, including work. The treated area may still feel slightly tender to touch, but this doesn’t typically interfere with daily life. Remember to continue avoiding sexual activity for the full 3-4 day period.

What Makes the Experience at Biltmore Restorative Medicine Different

The technical steps of the O-Shot are similar across providers, but the experience can differ dramatically. What sets our practice apart:

Personalized Attention: We never rush. Your comfort, both physical and emotional, takes priority over efficiency. We take whatever time needed to ensure you feel cared for and at ease.

Comprehensive Approach: We view the O-Shot as one component of sexual wellness, not an isolated procedure. Our evaluation and treatment planning consider your complete health picture.

Quality Standards: From our PRP processing to our sterile technique to our follow-up care, we maintain rigorous quality standards that produce exceptional outcomes.

Experienced Expertise: Experience matters enormously in technique refinement and patient care. Our extensive experience with the O-Shot means smoother procedures and better results.

Ongoing Support: We remain accessible and engaged with your progress. Questions or concerns after your procedure receive prompt, thoughtful responses.

Managing Your Expectations

Understanding that the O-Shot is a regenerative treatment, not an instant fix, helps set appropriate expectations. You won’t leave the office with immediate results—the magic happens over the following weeks and months as your tissue regenerates.

Most women begin noticing improvements within 3-6 weeks, with effects continuing to develop for 2-4 months. This gradual enhancement reflects true tissue healing rather than temporary effects.

Some women feel discouraged if they don’t notice immediate changes. Remember: PRP is stimulating collagen production, blood vessel formation, and nerve regeneration—processes that take time. Patience during this period allows the treatment to work its regenerative magic fully.

Your Journey to Renewed Wellness

The O-Shot procedure itself is relatively brief and comfortable, but it initiates a profound journey of tissue regeneration and restoration. Understanding each step of the process helps you approach your procedure day with confidence rather than anxiety.

At Biltmore Restorative Medicine, we’ve refined every aspect of the O-Shot experience to maximize both your comfort and your results. From the comprehensive consultation through the careful procedure and attentive follow-up, every element reflects our commitment to helping you feel like YOU again.

If you’re considering the O-Shot, I encourage you to schedule a consultation where we can discuss your specific situation and determine if this treatment aligns with your goals. Understanding what to expect is the first step toward the renewed sexual wellness you deserve.
